{"id":4595,"date":"2018-12-07T17:37:29","date_gmt":"2018-12-07T23:37:29","guid":{"rendered":"https:\/\/enchantedfamily.com\/blog\/how-to-obtain-records-of-birth-of-children-born\/"},"modified":"2018-12-07T17:37:29","modified_gmt":"2018-12-07T23:37:29","slug":"how-to-obtain-records-of-birth-of-children-born","status":"publish","type":"post","link":"https:\/\/ladykathleen.com\/family\/how-to-obtain-records-of-birth-of-children-born\/","title":{"rendered":"How to Obtain Records of Birth of Children Born&#8230;"},"content":{"rendered":"<p>\t\t\t\t<strong>Records of Children Born in Foreign Countries to<br \/>\nU.S. Parents<\/strong><\/p>\n<p align=\"left\">The<br \/>\nbirth of a child abroad to U.S. citizen parent(s) should be reported to<br \/>\nthe nearest U.S. Consulate or Embassy as soon after the birth as possible.<br \/>\nTo do this, the child&#8217;s parent or legal guardian should file an<br \/>\nApplication for Consular Report of Birth Abroad of a Citizen of the United<br \/>\nStates of America (Form FS-579\/SS-5). This form may also be used to apply<br \/>\nfor a Social Security Number for the child. A $40.00 fee is charged for<br \/>\nreporting the birth.<\/p>\n<p>The application must be<br \/>\nsupported by evidence to establish the child&#8217;s U.S. citizenship. Usually,<br \/>\nthe following documents are needed:<\/p>\n<ol>\n<li>the child&#8217;s birth<br \/>\ncertificate<\/li>\n<li>evidence of the U.S.<br \/>\ncitizenship of the parent(s) such as a certified copy of a birth<br \/>\ncertificate, U.S. passport, or Certificate of Naturalization or<br \/>\nCitizenship<\/li>\n<li>evidence of the<br \/>\nparents&#8217; marriage, if applicable<\/li>\n<li>affidavit(s) of the<br \/>\nphysical presence of the parent(s) in the United States<\/li>\n<\/ol>\n<p>Each document should be<br \/>\ncertified as a true copy of the original by the registrar of the office<br \/>\nthat issued the document. Other documents may be needed in some cases.<br \/>\nContact the nearest U.S. Embassy or Consulate for details on what evidence<br \/>\nis needed.<br \/>\nWhen the application is<br \/>\napproved, a Consular Report of Birth Abroad of a Citizen of the United<br \/>\nStates of America (Form FS-240) is given to the applicant. This document,<br \/>\nknown as the Consular Report of Birth, has the same value as proof of<br \/>\ncitizenship as the Certificate of Citizenship issued by the Immigration<br \/>\nand Naturalization Service.<br \/>\nA Consular Report of<br \/>\nBirth can be prepared only at a U.S. Embassy or Consulate overseas, and<br \/>\nonly if the person who is the subject of the report is under 18 years of<br \/>\nage when the application is made. A person residing abroad who is now 18<br \/>\nyears of age or over, and whose claim to U.S. citizenship has never been<br \/>\ndocumented, should contact the nearest U.S. Embassy or Consulate for<br \/>\nassistance in registering as a U.S. citizen.<br \/>\nAs of November 1, 1990,<br \/>\nthe U.S. Department of State no longer issues multiple copies of the<br \/>\nConsular Report of Birth. However, a replacement Consular Report of Birth<br \/>\nmay be issued if the original document is lost or mutilated. The U.S.<br \/>\nDepartment of State also issues certified copies of the Certification of<br \/>\nReport of Birth (DS-1365), which contains the same information as on the<br \/>\nConsular Report of Birth. The DS-1365 serves most needs and can be issued<br \/>\nin multiple copies. Documents are issued only to the subject of the<br \/>\nConsular Report of Birth, the subject&#8217;s parents or legal guardian, or a<br \/>\nperson who submits written authorization from the subject.<br \/>\nTo request copies of the<br \/>\nDS-1365 or a replacement FS-240, write to Passport Services,<br \/>\nCorrespondence Branch, U.S. Department of State, 1111 19th Street NW,<br \/>\nSuite 510, Washington, DC 20522-1705. Please include the following items:<\/p>\n<ol>\n<li>the full name of the<br \/>\nchild at birth (and any adoptive name)<\/li>\n<li>the date and place of<br \/>\nbirth<\/li>\n<li>the names of the<br \/>\nparents<\/li>\n<li>the serial number of<br \/>\nthe FS-240 (if the FS-240 was issued after November 1, 1990)<\/li>\n<li>any available passport<br \/>\ninformation<\/li>\n<li>the signature of the<br \/>\nrequestor and the requestor&#8217;s relationship to the subject<\/li>\n<li>a check or money order<br \/>\nfor $40.00 for the FS-240, $20.00 for the first DS-1350 and $10.00 for<br \/>\neach additional issued at the same time per document requested, made<br \/>\npayable to the U.S. Department of State<\/li>\n<li>if applying for a<br \/>\nreplacement FS-240, a notarized affidavit by the subject, parent, or<br \/>\nlegal representative that states the name, date and place of birth of<br \/>\nthe subject, and the whereabouts of the original FS-240<\/li>\n<\/ol>\n<p>To obtain a Consular<br \/>\nReport of Birth in a new name, send a written request and fees as noted<br \/>\nabove, the original (or replacement) Consular Report of Birth, or if not<br \/>\navailable, a notarized affidavit about its whereabouts. Also, send a<br \/>\ncertified copy of the court order or final adoption decree which<br \/>\nidentifies the child and shows the change of name with the request. If the<br \/>\nname has been changed informally, submit public records and affidavits<br \/>\nthat show the change of name.<\/p>\n<hr width=\"80%\">\n&nbsp;<strong>records of U.S. citizens who die in foreign countries<\/strong><br \/>\n<strong><br \/>\n<\/strong>The death of<br \/>\na U.S. citizen in a foreign country may be reported to the nearest U.S.<br \/>\nconsular office. If reported, and a copy of the local death certificate<br \/>\nand evidence of U.S. citizenship are presented, the consul prepares the<br \/>\nofficial Report of the Death of an American Citizen Abroad&#8217; (Form OF-180).<br \/>\nA copy of the Report of Death is then filed permanently in the U.S.<br \/>\nDepartment of State (see exceptions below).<br \/>\nTo obtain a copy of a<br \/>\nreport filed in 1963 or after, write to Passport Services, Correspondence<br \/>\nBranch, U.S. Department of State, Washington, DC 20522-1705. The fee for a<br \/>\ncopy is $20.00 for the first copy, $10.00 for each additional copy. Fee<br \/>\nmay be subject to change.<br \/>\nReports of Death filed<br \/>\nbefore 1963 are maintained by the National Archives and Records Service,<br \/>\nDiplomatic Records Branch, Washington, DC 20408. Requests for such records<br \/>\nshould be sent directly to that office.<br \/>\nReports of deaths of<br \/>\npersons serving in the Armed Forces of the United States (Army, Navy,<br \/>\nMarines, Air Force, or Coast Guard) or civilian employees of the<br \/>\nDepartment of Defense are not maintained by the U.S. Department of State.<br \/>\nIn these cases, requests for copies of records should be sent to the<br \/>\nNational Personnel Records Center (Military Personnel Records), 9700 Page<br \/>\nAve., St. Louis, Missouri 63132-5100.<\/p>\n<hr width=\"80%\">\n<strong>of birth and death occurring on vessels or aircraft on the high seas<\/strong><br \/>\n<strong><br \/>\n<\/strong>When a birth<br \/>\nor death occurs on the high seas, whether in an aircraft or on a vessel,<br \/>\nthe record is usually filed at the next port of call.<br \/>\n<b>1.<\/b> If the vessel or<br \/>\naircraft docked or landed at a foreign port, requests for copies of the<br \/>\nrecord may be made to the U.S. Department of State, Washington, DC<br \/>\n20522-1705.<br \/>\n<b>2.<\/b> If the first port of entry was in the United States, write to<br \/>\nthe registration authority in the city where the vessel or aircraft docked<br \/>\nor landed in the United States.<br \/>\n<b>3.<\/b> If the vessel was of U.S. registry, contact the local<br \/>\nauthorities at the port of entry and\/or search the vessel logs at the U.S.<br \/>\nCoast Guard Facility at the vessel&#8217;s final port of call for that voyage.<br \/>\n&nbsp;<\/p>\n<hr align=\"center\" width=\"50%\">\n<p align=\"center\">&nbsp;<\/p>\n<p align=\"center\"><i><\/i><!-- Newsletter Signup Box Top --><\/p>\n<p><!-- Additional elements such as Live Links and newsletters subscription boxes should follow the bottom navigation on your articles --><\/p>\n<p align=\"center\">Information is provided courtesy of the<br \/>\nNational Center for Health Statistics (NCHS)<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Records of Children Born in Foreign Countries to U.S. Parents The birth of a child abroad to U.S. citizen parent(s) should be reported to the nearest U.S. Consulate or Embassy as soon after the birth as possible.
